About Us

Clarity Health Systems is an innovative health-tech start-up developing solutions to help health and social care commissioners streamline operational delivery whilst ensuring that the individual at the heart of the process is involved and empowered throughout their care journey. We are experts in All Age Continuing Care (AACC) and are committed to partnering with Integrated Care Boards and Adult Social Care services across England.

We develop dynamic software applications built on our extensive knowledge and experience of AACC/NHS Continuing Healthcare (CHC). We support the end-to-end delivery of the NHS Continuing Healthcare process, providing transparency, and enhancing the patient and family experience—ensuring a simple and seamless process.

As Integrated Care Systems (ICSs) and Integrated Care Boards (ICBs) become established, we believe it is vital for system partners across health and social care to become better integrated through technological innovation, ensuring that the individual and their representatives are involved and empowered. Why Choose Us?

We started to work with ESG as a research and development partner for their Clarity Product earlier this year. From the very start they have been responsive to our requests and have continued to develop the product to meet our needs. They listen and respond to feedback and they have added improvements and resolved any issues quickly, whilst keeping us informed.

This year we have faced many challenges in relation to remote working and volume of work increases due to the Covid Hospital Discharge pathway and Discharge to Assess pathway impacting on our time. Clarity is therefore an important to tool to help us manage this increased workload.

Clarity has been easy for our booking team to use and the benefits that it can deliver to our service have been easy to explain. We are now able to see ‘at a glance’ what evidence has been gathered to support an assessment and chase any outstanding information. Our nursing homes have reported that it is easy to upload evidence to the system.

We are about to introduce our clinical staff to the assessment part of Clarity and are excited to embark on this stage of our work with them.

The key ingredient to making IT work for you in CHC is that you have a good relationship with the provider and work together to ensure that the product meets the needs of the service. We can safely say that ESG has met all our expectations in this regard and are forward thinking.
